3. Conference Request Form (including all required signatures & related Funding Source/Account Codes)
4. Conference registration
5. Conference flyer/brochure (if applicable)
6. Lodging/Travel/Meals (if applicable)

Important Details

• If requested amount is more than PDF guidelines allow, indicate additional source of funding (pre-approved district account code or personal funds.)
• Membership fees and college credit fees are NOT covered by PDF fund. However, college credit fees may be applied for via a Tuition Reimbursement Form.
• Submit required documents a minimum of two weeks before each PDF Committee Meeting  (30 days prior is strongly encouraged.)
• Date(s) of the conference must be a minimum of two weeks AFTER  the PDF Committee Meeting.
• Incomplete requests will be returned without action.
